Forensic Analytical Specialties, Inc. conducted Environment of Care (EC) Assessments and provided Statement of Conditions (SOC) Assistance to all VA VISN 21 Medical Centers in Northern California, Nevada, and Hawaii. Forensic Analytical provided these services predominately through Mock Joint Commission surveys. The primary focus of this project was to evaluate the VA’s current measure of compliance with the 2004 JCAHO standards and to assist each facility with preparation for its Joint Commission survey. The visits consisted of three major phases, including:

Review of plans, programs, and records;

Interviews with key managers of EC- and SOC-related subjects; and

Interviews and observations in clinical and support service areas of each medical center.
Forensic Analytical primary goal during the mock surveys and assessments was to look for deficiencies in documentation and training and then to help offer timely solutions.
